12 year-old Ethiopian boy presented with left flank swelling for the past 3 months. Maternal pregnancy course was not significant with a normal birth. Examination showed distended asymmetrical abdomen with absent rectus abdominis muscles, palpable tender left flank cystic mass, bilateral undescended testes and developmental hip dysplasia.
