 59 year old obese man presented with frequent urination and difficulty in emptying his bladder. His medical history is significant for type 2 diabetes and hypertension, and no surgical history. He noted a decrease in scrotal size after voiding and 2-stage voiding requiring pressure to the scrotum. Physical exam revealed the findings in the image. After further investigation, he underwent surgery.
